Application and payment of the fee

  • To take part in the forum please complete the attached application form and return it to the Organizing Committee
  • An invoice will be made out upon receipt of the duly filled application form and sent to the applicant by fax or E-mail
  • Summary
  • Summary of the presentation not exceeding four pages (Word Format: Times New Roman 14) to be published in the forum materials must be submitted to the Organizing Committee before November 15, 2013
